Spinal Cord Nuclei
The gray matter of the spinal cord is organized into longitudinal columns of cell bodies called nuclei. These nuclei process sensory afferents, coordinate autonomic outflow, and direct somatic motor output. Anatomist Bror Rexed mapped these cellular groups into ten cytoarchitectonic zones known as Rexed laminae (I through X), which directly correlate with the classical anatomical nuclei.
Posterior Horn (Sensory) Nuclei
The posterior (dorsal) horn receives primary afferent sensory fibers through the dorsal roots and contains sensory interneurons and projection neurons.
-
Marginal Zone (Lamina I / Nucleus Posteromarginalis):
- Location: Thin cap covering the apex of the dorsal horn along all spinal levels.
- Cell Types: Projection neurons and local interneurons.
- Input: Lightly myelinated A-delta and unmyelinated C fibers conveying noxious and thermal sensations.
- Output: Contributes axons to the ascending anterolateral system (spinothalamic tract) projecting to the thalamus.
-
Substantia Gelatinosa (Lamina II):
- Location: Dorsal horn cap just internal to Lamina I, present at all spinal levels.
- Appearance: Low myelin density, giving it a translucent gelatinous texture in fresh tissue.
- Function: Serves as the primary spinal gate for pain and temperature modulation. Interneurons release GABA, glycine, and endogenous opioids (enkephalins) to inhibit incoming nociceptive afferents.
-
Nucleus Proprius (Laminae III and IV):
- Location: Broad zone occupying the bulk of the dorsal horn head.
- Input: Heavily myelinated A-beta cutaneous sensory fibers carrying low-threshold mechanical sensations (discriminative touch, vibration, two-point discrimination).
- Function: Integrates cutaneous mechanoreception; axons cross the anterior white commissure to ascend in the spinothalamic system or project to adjacent motor pools.
-
Posterior Thoracic Nucleus (Clarke Column / Lamina VII):
- Location: Medial base of the dorsal horn, restricted to segments C8 down to L2-L3.
- Function: Major relay station for non-conscious proprioceptive information from lower limb muscle spindles and Golgi tendon organs.
- Output: Axons project uncrossed to form the dorsal (posterior) spinocerebellar tract, ascending to the cerebellum via the inferior cerebellar peduncle.
Intermediate Zone and Lateral Horn (Autonomic and Interneuronal) Nuclei
The intermediate gray matter bridges sensory and motor pathways and houses preganglionic autonomic neurons.
-
Intermediolateral Nucleus (IML / Lamina VII):
- Sympathetic Division (T1-L2): Forms the prominent lateral horn in thoracic and upper lumbar cord. Houses preganglionic sympathetic cell bodies whose axons exit through the ventral roots into white rami communicantes.
- Parasympathetic Division (S2-S4): Located in the lateral part of the sacral intermediate gray (often without a distinct lateral horn bulge). Houses sacral preganglionic parasympathetic cell bodies that form the pelvic splanchnic nerves.
-
Intermediomedial Nucleus (IMM / Lamina VII):
- Location: Positioned along the medial aspect of the intermediate zone adjacent to the central canal.
- Function: Receives visceral sensory afferents from internal organs and relays signals to the IML, closing spinal visceral autonomic reflex arcs.
-
Central Gelatinous Substance (Lamina X):
- Location: Gray matter encircling the central canal (gray commissure).
- Function: Contains crossing decussating axons (anterior white commissure lies just ventral) and local interneurons that modulate visceral nociception and bilateral spinal reflexes.
-
Phrenic Nucleus:
- Location: Medial core of the anterior horn in cervical segments C3-C5.
- Function: Houses somatic lower motor neurons dedicated to the innervation of the diaphragm via the phrenic nerve.
-
Spinal Accessory Nucleus:
- Location: Dorsolateral border of the ventral horn spanning segments C1-C5/C6.
- Function: Gives rise to rootlets that ascend through the foramen magnum and exit the jugular foramen as the spinal accessory nerve (CN XI), innervating the sternocleidomastoid and trapezius muscles.
-
Onuf Nucleus (Nucleus of Onufrowicz):
- Location: Ventral aspect of the anterior horn in sacral segments S1-S3 (concentrated at S2).
- Function: Specialized motor nucleus innervating striated sphincters of the pelvic floor, specifically the external urethral sphincter and external anal sphincter. Controls urinary and fecal continence.
Anterior Horn (Somatic Motor) Nuclei
The anterior (ventral) horn contains alpha and gamma lower motor neurons (Lamina IX) embedded in a matrix of interneurons (Lamina VIII) such as Renshaw cells. These motor neurons are organized into distinct topographical columns:
-
Medial Motor Column:
- Extent: Present along the entire length of the spinal cord (C1 through Co1).
- Innervation: Axial, paravertebral, and intercostal musculature responsible for maintaining posture and trunk stability.
-
Lateral Motor Column:
- Extent: Present exclusively within the cervical (C4-T1) and lumbosacral (L1-S3) enlargements.
- Innervation: Appendicular muscles of the upper and lower extremities.
- Somatotopic Arrangement:
- Ventral/Anterior neurons innervate proximal limb flexors and extensors.
- Dorsal/Posterior neurons innervate distal limb flexors and extensors.
-
Central Motor Column:
- Extent: Intermittent cervical and lumbosacral cell clusters.
- Components: Includes specialized motor pools such as the phrenic nucleus and the lumbosacral accessory motor pools.
